Hudson Meadow Senior Services is a locally owned, private-pay support service helping older adults, vulnerable adults, and Veterans remain safe and stable in the community.

We are not a home care agency.
We do not provide hands-on personal care, skilled nursing, or medical services.

Instead, we offer non-medical support, companionship, supervision, transportation, and care-navigation services that complement existing family support, public programs, and licensed providers.

Veterans and their families are a priority population within our organization.

Non-Medical Transportation & Escort

What We Do NOT Provide

• No hands-on bathing, dressing, or toileting

• No medication administration

• No medical or skilled nursing services

• No certified home health aide services

Our services are flexible, personalized, and designed to provide additional stability and support when informal supports are limited.

We frequently collaborate with families, providers, and community agencies to reduce crisis situations and improve continuity of care.

Hudson Meadow Senior Services welcomes collaboration with APS, Office for the Aging, Veterans’ services, social workers, and discharge planners seeking supplemental private-pay support for clients who do not require licensed home care.

Medicaid Long-Term Care (LTC) Planning Support

 

Navigating Medicaid and long-term care services can feel overwhelming. The paperwork, eligibility rules, and coordination between providers can quickly become confusing for families.

Hudson Meadow Senior Services offers guidance and structured support to help families understand their options and move through the process with greater confidence.

Our Medicaid LTC Planning Support Includes:

• Assistance gathering required documentation

• Help organizing medical and financial records

• Coordination with Medicaid planners, elder law attorneys, and caseworkers

• Support during the application and recertification process

• Education on Medicaid Managed Long-Term Care (MLTC) programs

• Referrals to trusted local community resources

We do not provide legal advice or act as attorneys.
Our role is to assist with preparation, coordination, organization, and ongoing support to help streamline the Medicaid process.
